Summary

This study examined 10,103 patients with chronic obstructive pulmonary disease (COPD) who transitioned from dual therapy to triple therapy budesonide/glycopyrronium/formoterol (BGF) following an acute exacerbation. Triple therapy refers to the combination of three medications in one inhaler: budesonide (a corticosteroid that reduces inflammation), glycopyrronium (a long-acting muscarinic antagonist that helps open the airways), and formoterol fumarate (a long-acting beta agonist that helps relax the muscles around the airways). Patients were grouped based on how quickly they started BGF: within 30 days (prompt), between 31 and 180 days (delayed), between 181 and 365 days (very delayed).

The findings revealed that prompt escalation to triple therapy was associated with significantly fewer COPD exacerbations and severe cardiopulmonary events, including mortality, heart failure hospitalizations, myocardial infarctions, and cardiac arrests. Specifically, patients in the prompt group had 23% and 31% fewer exacerbations, and 19% and 13% fewer severe cardiopulmonary events compared to those in the delayed and very delayed groups, respectively. These results underscore the importance of early intervention in reducing the health risks and complications associated with COPD.

Why this matters

This research showed that earlier escalation to triple therapy (BGF) after a COPD exacerbation was linked to lower rates of future exacerbations and serious cardiopulmonary events, including life-threatening outcomes. While these findings suggest that timely treatment could significantly improve patient outcomes and reduce healthcare utilization, it’s important to note that this was a retrospective, observational study. More research is needed to confirm these benefits and guide best practices for early therapeutic intervention in COPD management.
