No More Guesswork: Regional One Health Transforms Patient Safety

At Regional One Health, the clinical pharmacy team was managing a growing population of high-risk patients, but with limited visibility and mounting risk. Manual tracking methods couldn’t keep pace, and their EHR offered no reliable way to identify patients needing follow-up, labs, or dose adjustments.
“We were flying blind before,” said Jennifer Campbell, Clinical Pharmacist in Ambulatory Care at Regional One Health. “Now we can see exactly who needs attention and act fast.”
That clarity came with the implementation of Inovalon’s VigiLanz Pharmacy Surveillance Pro™. What began as an effort to improve anticoagulation safety soon turned into a broader transformation in clinical efficiency, pharmacist satisfaction, and provider education.
From risk to real-time oversight
Before Inovalon, anticoagulant oversight was a daily challenge. Patients prescribed DOACs (direct oral anticoagulants) by medical residents were not consistently monitored, and the pharmacy team lacked a dependable system to catch safety issues.
“We had people in clinic being prescribed DOACs by their medical residents without appropriate monitoring or oversight,” Campbell explained. “The pharmacy team had no reliable system to catch them and no time to build one.”
Anticoagulants, commonly known as blood thinners, are among the top contributors to medication-related emergency visits, and without better visibility, the danger was real. That’s when Regional One Health’s pharmacy team partnered with Inovalon.
Using VigiLanz Pharmacy Surveillance Pro, the team configured a custom set of surveillance rules. Now, every day begins with an automatically generated worklist that highlights missed labs, overdue appointments, questionable dosing, or potentially risky medication combinations.
“It’s become part of our daily routine,” Campbell said. “We don’t wait on monthly reports. We can act now.”
Faster follow-up, fewer gaps
Pharmacists no longer need to dig through charts or spreadsheets to figure out who needs attention. With near real-time alerts, they intervene sooner, prevent harm, and free up more time for patient care.
“It saves at least 30 minutes a day (per pharmacist) and helps us act before a problem becomes serious,” Campbell said.
The system didn’t just improve efficiency – it strengthened care delivery. Patients were no longer slipping through the cracks. Interventions happened faster. And with every documented touchpoint, pharmacists helped educate medical residents on safer prescribing practices.
“We’re not just improving care; we’re teaching future physicians what good stewardship really means,” Campbell noted.
Laying the groundwork for broader risk management
While the initial goal was improving anticoagulation safety, the pharmacy team quickly saw the potential to extend rule-based monitoring to other high-risk medications and care settings – laying the foundation for broader risk management.
“This isn’t just about one medication class,” Campbell said. “It’s how we’re managing risk across the board.”
With Inovalon’s configurable solution, Regional One Health has the tools to scale their surveillance practices and proactively manage patient safety in ways they couldn’t before.
