Rethinking Clinical Trial Recruitment: How Technology is Expanding Access to Clinical Trials

Quick Navigation

Despite years of innovation, patient recruitment remains one of clinical research’s toughest – and most persistent – challenges. Trials stall. Timelines slip. Not because science is failing, but because recruitment is stuck using outdated tools to solve modern problems.

Today’s studies are more complex, personalized, and inclusive by design. For clinical trial teams to keep up, they must move beyond traditional models and embrace recruitment that’s faster, more flexible, and built for real people.

In a recent interview with Applied Clinical Trials, Cameron Breze, MS, Product Manager of Clinical Insights at Inovalon, shared where traditional recruitment is falling short – and what it takes to get it right

Old models, new problems

For decades, clinical trial recruitment has relied on academic medical centers and a few high-performing site coordinators. That model worked when trials were simple and broadly targeted. But now, trials often come with dozens of inclusion and exclusion criteria. And the ‘ideal’ participant isn’t always in the waiting room.

“Some trials are for very, very specific types of cancer or genetic diseases,” says Breze. “Getting a patient who meets the full set of criteria – and is willing to participate – is a real challenge.”

Many patients face barriers that prevent them from even being considered: lack of transportation, rigid work schedules, limited access to specialists. Reaching more diverse and representative populations requires greater flexibility in trial offerings – especially those that expand access to patients outside of traditional care settings.

Technology that’s built for people

Automation and AI are reshaping clinical trial recruitment, making it possible to match patients faster, minimize manual errors, and reduce site burden. But the technology only works if people use it.

“There’s always a hesitation with doctors, with clinical researchers, who are already overloaded, to learn a more complex tool,” Breze explains.

That’s why recruitment platforms must follow the lead of consumer apps – intuitive, helpful, and straightforward. The focus needs to shift from just adding features to designing tools that actually simplify real-world workflows.

Better data, better outcomes

Precision recruitment doesn’t just need more data – it needs better data. As Breze puts it, “Garbage in, garbage out.” Without complete, reliable data, even the most sophisticated tools can misfire.

Breze points to a critical care study where fluid input was carefully tracked – but output was missing. That single data gap made a key model ineffective. Multiply that across thousands of patients, and it’s clear: poor data isn’t just a technical problem, it’s a clinical one.

Improving outcomes means verifying data sources, ensuring completeness, and aligning clinical and technical teams from the start.

Decentralization is not one-size-fits all

The rise of decentralized and hybrid trial models has opened new doors for patient access and retention. Trials that require simple interventions – like dermatology or mental health studies – can often be conducted remotely.

But not every trial fits that mold. “There are more complex biologics… where they’re required to be administered through an IV,” says Breze. “Those will always need onsite care.”

The most forward-thinking organizations aren’t choosing between decentralized or site-based; they’re building flexible, hybrid strategies that adapt to the needs of each study.

The future of recruitment: scalable, streamlined, and human

Patient recruitment isn’t just about enrolling faster – it’s about making trials more inclusive, more efficient, and more human.

As Breze puts it, “We’re structuring our efforts in a way that are more scalable, reproducible, and streamlined for everyone involved.”

That’s the future: not just high-tech, but high-trust.
